Transfer Providers
We recognize that your first provider may not be the right match for you, and this is more than okay. We want you to find a provider who makes you feel supported and will help you reach your mental health goals.
If you would like our care team to help match you with another provider, please email support@3yhealth.com or call 346-512-0464.
If you would like to book directly with a new provider, please follow the steps below.
Login to your Client Portal at clients.3yhealth.com.
Click on the “+ Schedule now” button on the top right hand side of the page
Choose “+ Book with a new provider”
You may be prompted to enter your state and insurance information to find a new provider. Once you select the provider, all of your information will transfer over to your new provider.
After scheduling your appointment, you do not need to fill out intake forms again, as your new provider will have access to your original intake forms. You are all set!
