All onboarding forms must be completed prior to your first session with 3Y Health Mental Health. This step is essential to ensure you receive the highest quality care tailored to your unique needs.

To complete your onboarding forms, please follow the instructions below—it only takes a few minutes!

If you haven’t done so already, create your account or log in at clients.3yhealth.com using the email address that you booked with.
Once logged in, you’ll find your onboarding forms listed under “Tasks: Start Onboarding Here” on the left-hand side of your dashboard.
Please complete all forms - this should take no more than 8 minutes!

Your progress will be saved as you complete each form. You can start, pause, and resume as needed. However, all forms must be completed prior to your first appointment.
